Nitrous Oxide - November 2023

From 8th November 2023 Nitrous oxide has been classified as a controlled Class C substance under the UK Misuse of Drugs Act (1971). Nitrous oxide can continue to be used for legitimate industrial, medical and veterinarian purposes. VBS supplies proprietary CryoPen N02 cartridges for use in CryoPen Cryotherapy devices. VBS Direct Ltd will only supply Nitrous oxide to clients for use in Cryotherapy devices and so with a legitimate business use in line with the legalisation.

Our clients should store the cartridges with the usual safeguards they have in place for medicines and other controlled products to ensure they are not accessed by the public and are solely used for legitimate purposes.

What to treat

Smaller lesions on the face, nose, eyelids, ears, legs and feet are often considered not important enough for surgery under general or local anesthetic. CryoPen offers an innovative solution in these cases.

Pets can be in and out in just minutes and they emerge bright and bouncy, rather than having to be monitored for two to three hours.

There’s no cutting or bleeding. Therefore, nothing for your patient to lick or pull on. Minimizing risk of infection.

How to Treat

The freezing of the tissue is quick. Treatments only take seconds, with no anesthesia or pre or post-op care required. The CryoPen works with very practical disposable cartridges and emits a fine jet of nitrous oxide under high pressure.

Thanks to the extreme precision there will be no collateral damage to healthy tissue. Load the device, check out the selection of applicators allowing for the treatment from a pinpoint 1mm up to a width of 20mm, point and freeze…

Cryosurgery, of course, is nothing new. It is recognized as THE reference in the dermatology clinic. However, veterinarians who have struggled with cryoliquid spills, imprecise cotton buds and laborious deliveries of hazardous liquid nitrogen will experience a different technology by using the CryoPen.

A Veterinary Perspective

cyropen-alancochrane.jpg

Compared to the alternative, which is surgery under either general or local anesthetic, freezing the tissue is much quicker with the CryoPen. It’s so incredibly accurate and practical. The freezing penetrates 1 millimeter for every 5 seconds up to a depth of 7mm in one treatment session. Pets can be in and out in just minutes and they emerge bright and bouncy, rather than having to be monitored for two to three hours after surgery and returning home groggy. There’s no cut, no bleeding and no sutures, so nothing to lick or to pull at or the risk of infection either.

  6. Point to target tissue of lesion and freeze for allotted time ( stop freezing if patient indicates start of burning pain)
  7. Let area thaw (minimum of 30 secs)

Top Tips

  • Clip hair away from lesion margins to help with exposure if appropriate.
  • Cover eyes with lube and protective dampened gauze swab (with H20) if working in the facial region.
  • When treating Distichiasis – Ensure eyelid is rotated outwards to visualise, use ophthalmic loupes if available either treat individual hair follicles independently or for multiple hairs, treat the eyelid hair line completely and then remove the hairs with forceps. Protect the eye itself and provide lube for post op, there is likely to be increased erythema and swelling of eyelids temporarily for 24-48 hrs.
  • Keep Cryopen in a 45-90 Degree angle to allow correct expulsion of propellant.
  • Pre warn owners that larger broad-based lesions may require multiple treatments to debulk initially to remove gross material and to penetrate to base of lesion.
  • With pedunculated lesions treat the stalk base only.

How to change a Cartridge on your CryoPen B+

Remove the old cartridge by un-screwing it. Then tip out the used nib from inside the CryoPen. Using the tweezers provided with the CryoPen place the new nib correctly in the base of the CryoPen with the point facing upwards.

Screw the cartridge into the CryoPen so that you first hear the point of the nib puncture the cartridge. Continue to screw the cartridge until it is impossible to screw any further. Failure to screw tightly enough will result in the leakage of gas. When removed the fully discharged metal cartridges can be recycled along with other metal waste

Do not keep in a room with large temperature variations, or in the back of a car.

CryoPen Device Cleaning Guidance

It is recommended to clean the CryoPen between uses. The device cannot be immersed, soaked or rinsed in disinfecting liquids as the inner switches may be damaged. The recommended procedure to clean the device is as follows:

  • Soak a wipe with a noncorrosive disinfecting solution.
  • Apply the wet wipe on the surface of the instrument and to the applicator tips (removing cap first).
  • Recommended contact duration: 1 to 5 min. Always follow the recommendation of the disinfecting solution manufacturer with regards to the concentration and the time of application.

Sterilization of CryoPen Applicator Tips

Neither the CryoPen device, nor the micro-applicator tips should be immersed in liquids. However, CryoPen micro-applicator tips may undergo a steam sterilization process.

The two most common steam-sterilizing temperatures are 121°C (250°F) and 134°C (273°F). These temperatures (and other high temperatures) must be maintained for a minimal time to kill microorganisms. Recognized minimum exposure periods for sterilization of wrapped healthcare supplies are 20 minutes at 121°C (250°F) in a gravity displacement sterilizer or 4 minutes at 134°C (273°F) in a prevacuuum sterilizer:

  • Remove the micro-applicator from the device and remove the applicator cap.
  • Place the cap and micro-applicator on their largest base in the appropriate trays/baskets.
  • Follow the instructions manual provided with your sterilizer manufacturer to choose and launch the sterilization process.
